VCU School of Medicine evaluators, thank you for evaluating our students. 

Please select the entrusted professional activity (EPA) that you directly observed.  A new form must be completed for each direct observation. After submitting, evaluators will receive a copy of the evaluation results and can then choose to forward these results to the student via email.  Note: Emailed results are best viewed and forwarded from the Outlook Web App or Outlook Mobile App, not Outlook for PC or Mac.
